Prospective Student Advisor Preference Form

What is this form for? If you are applying to, or considering applying to, the LT PhD program, this form tells us which faculty members you would prefer to work with. There are two different roles to rank:

You'll rank three choices for each role. We ask for three because a faculty member's first choice may already have a full roster of students; staff will contact your top choice first and only move down the list if needed.

AGF mentor preference

In addition to a faculty advisor/major professor who works with students on portfolio and dissertation, each student is also assigned an Associate Graduate Faculty (AGF) mentor. Rank your top three choices below, in order of preference. Department staff will contact your first choice to confirm availability; if unavailable, staff will proceed to your second and then third choice.

Each AGF choice must be different.

Major professor preference

Please rank your top three preferred Major Professors (faculty advisors) from the regular Learning Technologies faculty, in order of preference. Department staff will contact your first choice to confirm availability; if unavailable, staff will proceed to your second and then third choice.
